The Puchiguso Hotaru carries around all over the place is very important to her, and also plays a key role in Hotaru's first appearance.
In the manga, Hotaru walks up to Rena and Shugo, but doesn't mention that she needs help in curing its sickness until Rena points it out. In a rude interference from Komiyan, he mentions that the little Puchiguso will die in about an hour from then, and a couple of newbies like them wouldn't be able to afford some Phoenix Down to cure it. Rena got angry at Shugo and logged out, but Shugo didn't want to leave Hotaru alone and offered to help her out. After learning the keywords to find some Phoenix Down, Shugo and Hotaru went to that area. After fighting off Cerberus, Shugo was able to obtain the Phoenix Down and heal the sick Puchiguso, much to everyone's disbelief. So all ended happy and Hotaru has tagged along ever since.

In the anime, however, Hotaru was just minding her own business one evening when a very familiar face comes along, asking Hotaru to take care of her baby Puchiguso. Hesitant to do so, Hotaru took up the job to take care of it. Later, when Shugo coincedentally sits next to Hotaru in Mac Anu, he notices that the Puchiguso is sick, and Hotaru doesn't know the cure for it. A Ranch Owner explains what they must find and where they could find it, but the level of the area is way out of their league. Shugo insists on going anyway. They managed to avoid monsters in the field and half way through the dungeon, but soon begun to get chased. Sanjuro pops up and helps them fight off the monsters, the cure is found, Puchiguso feels better, and all ends happily.
